If you or a loved one is interested in our program, please begin by filling out our application. This initial step collects essential details about the applicant. The more information you provide, the better we can assess if our program is the right fit. Once submitted, we will review your application and get in touch.

After reviewing your application, if we believe our
program could be a good fit, we will request pertinent medical records. At this stage, we will also ask you to submit a personal narrative. This narrative is crucial, as clinical records alone can’t capture the full picture. Sharing a personal summary from the family’s or the applicant’s perspective helps us get to know and understand the individual beyond their medical history.

If both we and the applicant agree our program is a good match, we will then arrange either a visit or an admission date. A virtual visit via Zoom may be the preferred option in some cases, while in others, the applicant may stay for a few days to experience our program firsthand. We understand that not everyone can visit, and that’s perfectly fine too.
